From the read me file:
Address Book is an easy-to-use, yet fast and powerful way to provide you with complete management and control of lists of names, addresses, and phone numbers. It comes in application and Desk Accessory* versions and works on Macintosh computers from the Mac Plus to the latest PowerMacs.
*Note: The archive from the magazine cover disk does not contain the Desk Accesory version.
Address Book has various print options for labels, envelopes, Rolodex cards, etc. It does not have a field for email addresses or websites, other than typing them in the Notes field.
When finished viewing a record's detail, don't click on the window close box - that quits Address Book. Instead, click on the "Index" button in the lower-right-hand corner to return to the alphabetical list view.
The application is on a Disk Copy 4.2 disk image (1.44MB) inside a StuffIt 3.5 archive.
